Immunization Requirements

The Nurse/Health Assistant in each building annually reviews the immunization records provided by the parent or previous school to ensure the proper immunization of all students in accordance with Ohio Law. If it is determined that the student needs additional immunizations, it is expected that immunizations are obtained immediately upon notification from the Health Office.

If a student is found to be out of compliance with Ohio's Immunization Law, the student may be excluded from school after 14 days into the school year per Ohio law.

Missed school days for immunization non-compliance are counted towards truancy.  

In an attempt to avoid exclusion, letters to home and phone calls will be made. Please ensure your contact information is accurate and up to date.

In Ohio, a parent/guardian has the right to request an exemption from immunization to one or more vaccinations based on medical, religious or philosophical reasoning. The form is available below, and must be completed each school year that the child is enrolled. Immunization Exemption Request 

Where you can get immunizations

NWLSD is
not a vaccination provider. If you need guidance on locations where you can get your child vaccinated, here are some suggested providers:

  • Your child’s primary care physician (schedule a well-child visit, be sure to make appointments well in advance).

  • Local clinics/community health centers/health departments (Click Here) offer free or sliding scale services.

  • Pharmacies- Walgreens, CVS and Kroger Little Clinics all offer vaccine services at cost.
