Northwest Local School District (NWLSD) utilizes an online enrollment system to make the student enrollment process more efficient and accessible for families. To get started, families are required to complete the online enrollment application and upload all necessary supporting documents.

BEGIN ENROLLMENT PROCESS

For NEW students registering for the 2024-25 school year and the  2025-26 school year, follow steps below.

STEP 1: Verify you live within NWLSD attendance boundaries:  Locate School / Boundary and Transportation Eligibility

STEP 2: Gather the required documents necessary for school enrollment:  

Have the following documents readily available and ready to upload:

  • Parent/guardian government-issued photo ID

  • Original birth certificate

  • Proof of custody documents

  • Proof of residency documents

  • Student information -- demographic and health/medication information.

  • Parent information -- work and cell phone numbers, email addresses.

  • Emergency contact -- phone numbers

Preschool Registration for 2024-25 is available by packet only. Click (HERE) to download our preschool enrollment registration packet. All families must schedule an appointment. Appointments can be made by calling 513- 385-8000. All preschool registration packets must be filled out and returned in-person.

STEP 4: Once all information has been successfully submitted, please monitor your email for updates on the status of your application—whether it is approved, denied, or placed on hold. If additional information or documentation is required, a representative from the Enrollment Office will reach out to you.

Enrollment Documents You Will Need

Required documents to enroll a student (items in bold are required): 

  • Parent/Guardian Current Photo ID

  • Name and Address of previous school

  • Birth Certificate, Passport, Hospital Birth Records, Immigration Documents - required for all students.  An ORIGINAL birth certificate is required for all Kindergarten students and students enrolling in an Ohio school for the first time. (Kindergarten students must be 5 by September 30, 2024)

  • Proof of Custody - proof of marriage, Custody agreement/court order if separated or divorced, court-ordered guardianship if student resides with someone other than biological parent, Grandparent Power of Attorney. 

  • Proof of Residency - business mail (delivered by USPS within last 30 days) for homeowners.  Current commercial lease if renting from a company. If renting private property, current notarized residency affidavit with owners' proof attached and current business mail (delivered by USPS within last 30 days) in the name of the parent/guardian. No online document accepted for proof of residency! (Please refer to the checklist at the end of the online registration for specific documents required.)

  • Current report card for grades 1-9. 

  • Transcript and/or current schedule from previous school is mandatory for 10th, 11th and 12th graders.

  • Health Records received from the previous school will be reviewed to keep within the guidelines for school enrollment.

  • Copy of the most recent IEP, MFE/ETR or 504 Plan (If applicable)
